Short answer: Exposure and response prevention (ERP) is a form of cognitive behavioral therapy and the first-line, best-supported treatment for OCD. You gradually face the thoughts or situations that trigger your obsessions, while resisting the compulsions you'd normally use to relieve the anxiety. Over time, the brain learns the anxiety fades without the ritual.
Why ERP works
Compulsions bring quick relief, and that relief teaches the brain the ritual is necessary. ERP breaks the loop. When you stay with the discomfort without performing the ritual, anxiety rises, peaks, and falls on its own. Each time, the obsession loses some of its power.
What ERP looks like step by step
- Assessment: you and your clinician map your obsessions, compulsions, and what you avoid.
- Building a hierarchy: a list of triggers ranked from mildly uncomfortable to very difficult.
- Exposures: starting low on the list, you face a trigger, in session and as practice between sessions.
- Response prevention: you resist the compulsion, including mental rituals and reassurance-seeking.
- Moving up: as anxiety drops, you move to harder exposures.
An example
Someone with contamination OCD might start by touching a doorknob and waiting 10 minutes before washing, then build toward not washing at all after ordinary contact. Someone with harm-related OCD might practice holding a kitchen knife while cooking without seeking reassurance.
How long does ERP take?
Many people see meaningful improvement in 12 to 20 sessions. Practice between sessions makes a big difference to results.
Is ERP hard?
It's challenging by design, but it's always collaborative and gradual. You never do an exposure you haven't agreed to. Most people find the discomfort is far less than the daily toll of OCD.
ERP and medication
For moderate to severe OCD, combining ERP with medication can improve results. Medication may lower anxiety enough to make exposures more manageable.
Frequently asked questions
Does ERP work for "Pure O" OCD? Yes. Exposures target the intrusive thoughts themselves, and response prevention targets the mental rituals.
Can ERP be done via telehealth? Yes. Many exposures can be done at home with a therapist guiding you on video, which can even make them more realistic.
What if I've tried therapy before and it didn't help? General talk therapy isn't the same as ERP. Many people improve once they get OCD-specific treatment.
